Provision of the information requested in this form is voluntary. The information is being collected by Service NSW on behalf of the Department of Family and Community Services (FACS) for the purpose of issuing a NSW Seniors Card or NSW Senior Savers Card and sending you Seniors Card Program communications, including special offers and discounts. Refusal to provide your information may mean that we cannot issue you with a Seniors Card or Senior Savers Card, or send you Seniors Card Program communications.
Your Medicare details and date of birth are being collected to determine your eligibility to obtain a Seniors Card or Senior Savers Card. Your Medicare card may have its authenticity verified through the National Document Verification System (DVS). Your Seniors Card Program status may also be verified by other government organisations, for example, Transport for NSW to determine your eligibility for a Senior Opal Card. Service NSW may also notify other government organisations of any changes to your eligibility for the Seniors Card Program.
Optional information about your pension status, country of birth, languages spoken or your Aboriginal or Torres Strait Islander status is used for statistical purposes only.
Your personal information may be disclosed:
- to external agencies contracted to manufacture the card or assist with mailing information about services and offers for Seniors Card and Senior Savers Card holders
- to verify the authenticity of your Medicare card through the National Document Verification System (DVS)
- to other government organisations for the purpose of verifying your Seniors Card Program status
- if required or authorised by law.
Your personal information will be held by FACS, Locked Bag 4028, Ashfield NSW 1800. You have the right to access and correct the personal information that FACS holds about you.
