Change the details of a food safety supervisor trainer

Introduction

To change the details of a food safety supervisor trainer in your registered training organisation (RTO), you need to send an application to the NSW Food Authority.

What you need

• your RTO approval number
• the updated details
• the trainer's resume, and certified copies of their:
  • food safety supervisor certificate
  • qualifications and certificates, as indicated on the form.

How to change

1. Gather the required documents.
2. Select the 'Download PDF form' button.
3. Complete and sign the form.
4. Submit your application by post or email, to the details on the form.

Applications will be processed within 4 weeks.

A ‘certified copy’ is a photocopy that has been compared with the original and endorsed as a true copy by a Justice of the Peace (JP), or a public officer such as a police officer.
