What you need

  • details of the affected egg brands and licensees
  • your contact details
  • a summary of the investigation
  • the PDF form – 'Notification of Equipment Failure – Egg Stamping'.

How to notify

  1. Select the 'Download PDF form' button.
  2. Complete the form.
  3. Submit your report to the NSW Food Authority.

More information

  • You must maintain a copy of the report in your Food Safety Program for further assessment by a Food Safety Officer.
  • A grace period from the stamping requirement will be provided for your business, in writing.
  • Operations will resume on a date determined by you and the NSW Food Authority, and unstamped eggs are not to be sold beyond this date unless an extension is granted.
