Change your name on a boat, vessel or other maritime licence record

Introduction

If you need to change your name on maritime-related products or services such as a vessel registration or your boat licence, you can update in person at a Service NSW Centre, or via mail.

What you need

• your proof of identity 
• proof of name change documentation
• the PDF form – 'Change of Records – Maritime Products Only'.

Note: If you hold a vehicle registration, NSW driver licence or combined licence and/or photo card, you need to complete the `Change of Records - Road and Maritime Products' form instead, and visit a service centre. 

How to change

1. Select the 'Download PDF form' button.
2. Complete the form.
3. Attach your supporting documents.
4. Provide the information:
   • in person at a service centre, or
   • via mail using the details on the form.
