Service NSW is collecting your personal and health information as part of exercising the customer service functions of Transport for NSW (TfNSW) for Mobility Parking Scheme (MPS) permit applications.

Service NSW and TfNSW are both subject to the Privacy and Personal Information Protection Act 1998 and the Health Records and Information Privacy Act 2002 when collecting your personal and health information.

Service NSW is collecting your personal information to:

  • perform its functions relating to the administration of the MPS
  • communicate with you about the progress and/or status of your application
  • verify your identity information through the Document Verification Service.

Personal information collected will include: your name, address and contact details; documents establishing your identity, details about your driver licence and licensing history, and medical information provided by your doctor which demonstrates any mobility disability that you have, e.g. a medical certificate.

Service NSW will disclose the information provided in your Mobility Parking Scheme permit application to TfNSW.

TfNSW is entitled to obtain your personal information for the Mobility Parking Scheme under the Road Transport (General) Regulation 2013 and you are required to provide this information under the Road Transport Act 2013 and the Road Transport (Driver Licensing) Regulation 2017, and your application may be refused if you do not provide it.

Service NSW will otherwise not disclose your personal information without your consent unless authorised by law.

Service NSW will take reasonable security measures to protect your personal information from loss, unauthorised access, use, modification, disclosure or other misuse. Your personal information will be held at Service NSW, Level 20, 2-24 Rawson Place, Sydney NSW, 2000, and disposed of securely.
