1. Definitions
1.1 The following definitions apply to these Terms and Conditions:
a. Agency means Service NSW.
b. Applicant means a person who is the holder of the Pre-IVF Fertility Testing Rebate form.
c. Ministry means the NSW Ministry of Health.
2. Application
2.1 These Terms and Conditions apply to applicants for the Pre-IVF Fertility Testing Rebate. By applying, you agree to be bound by these Terms and Conditions.
3. Eligibility Criteria
3.1 To be eligible for the Pre-IVF Fertility Testing Rebate:
a. You must be female.
b. You must be a resident of New South Wales.
c. You must provide proof of identity via your MyServiceNSW Account.
d. You must provide a receipt to confirm you have incurred out of pocket cost relating to pre-IVF fertility testing.
e. You must provide an endorsed Pre-IVF Fertility Testing Rebate form from your specialist to claim the rebate.
f. You must have incurred the eligible pre-IVF fertility test within Australia after 30 September, 2019.
g. The information you provide must be true and can be audited or investigated at any time by the Ministry.
h. You understand that it is an offence under the Crimes Act 1900 (NSW) to provide false or misleading information to a public authority
4. Conditions
4.1 For applications received before 31 December 2022, one $500 rebate is available per eligible applicant. For applications received on or after 1 January 2023, one $250 rebate is available per eligible applicant.
4.1.1 Applications must be made by the female who received the eligible pre-IVF fertility test.
4.2 Payment will be made via EFT to customer nominated bank account.
4.3 The Pre-IVF Fertility Testing Rebate is available only for eligible applicants.
4.4 The Agency reserves the right to decline to approve the payment of the Pre-IVF Fertility Testing Rebate for any reason.
4.5 The Ministry has the right, at its discretion, to cease payments of the Pre-IVF Fertility Testing Rebate at any time.
5. Verification
5.1 Service NSW and the Ministry has the right, at its discretion, to verify any information provided as part of the application. Verification may include, without limitation, any or all of the following:
a. Requesting the provision of additional information.
b. Confirmation that the Eligibility Criteria have been met.
5.2 Notwithstanding any other provision of these Terms and Conditions, the Agency reserves the right to require additional information or clarification from you, or anyone else before proceeding to make a decision about whether any applications are eligible or ineligible.
5.3 You understand that:
a. The details you have provided as part of the Proof of Identity process will be disclosed with the Department of Home Affairs for the purpose of verification.
b. This consent, once signed, remains valid while you are a customer of Service NSW unless you withdraw it by contacting Service NSW or the Department of Home Affairs.
c. You can get proof of your circumstances/details from the Department of Human Services and provide it to Service NSW so your eligibility for the Pre-IVF Fertility Testing Rebate can be determined.
d. If you withdraw your consent or do not alternatively provide proof of your circumstances/details, you may not be eligible for the Pre-IVF Fertility Testing Rebate provided by Service NSW.
