The Voucher profile is a voluntary feature in a MyServiceNSW Account.
Once a Voucher profile is created it can be used to help check a child’s eligibility for any vouchers that are made available through a MyServiceNSW Account.
Types of personal information we collect
To create the Voucher profile, Service NSW will collect personal information such as your child/ren's:
- name
- date of birth
- Medicare card details, such as the card number, individual reference number, full name and card type
- name of the educational institution the child/ren are enrolled in for 2023 or beyond
- primary residential address.
How we will use and disclose this information
Service NSW will use this personal information for the purposes of creating a Voucher profile in your MyServiceNSW Account. Once a Voucher profile is created you can use the information linked to the Voucher profile to check eligibility for any vouchers made available in a MyServiceNSW Account.
This function is optional. You can remove, add or update the child/ren's details from the Voucher profile at any time. You can also choose to remove the Voucher profile function from your MyServiceNSW Account.
Service NSW will not use or disclose your or your child/ren's personal information for any other purpose unless you provide your consent, or we are required or authorised to do so by law.
Whether you are required by law to provide us with this information
Providing us with the requested information for the purpose of creating a Voucher profile is voluntary. However, if you choose not to provide us with the requested information, you will not be able to create a Voucher profile.
