Introduction

If your name has changed or been altered in any way, you'll need to register a change of name with the Registry of Births, Deaths & Marriages. You can do this in person at a service centre, or submit your application by mail.

Eligibility

You're eligible to have your name changed if:

  • you're not a restricted person
  • your birth is registered in NSW
  • your birth is not registered in NSW and a protection order has been made to protect you and/or your children from domestic violence
  • you were born overseas and have been a resident in NSW for 3 consecutive years immediately prior to your application.

To register a change of name for a child under 18 years, both parents must make a joint application. If both parents cannot make a joint application, call 13 77 88 to discuss the circumstances.

What you need

For all applications:

  • the appropriate PDF form:
    • 'Application to register a change of name for an adult', or
    • 'Application to register a change of name for a child (under 18)'.
  • evidence of how your former name and current name were assumed
  • all identification and supporting documents certified by a registry approved witness (as indicated on the form)
  • your personal and contact details
  • evidence of your current residential address
  • your payment.

In addition:

For an adult born overseas:

  • evidence of your Australian citizenship or visa
  • evidence of your place of birth
  • 3 certified identification documents including a current photo ID
  • certified documentation that you have lived in NSW for the 3 consecutive years prior to your application.

For a child born overseas:

  • the personal and contact details of the parents
  • 3 certified identification documents for each parent, including photo IDs and evidence of their places of birth
  • evidence of the child's Australian citizenship or visa
  • the child's birth certificate
  • evidence of the child's place of birth
  • certified photo identification of the child
  • 2 other certified identification documents
  • certified documentation that the child has lived in NSW for the 3 consecutive years prior to the application.

For an adult born in NSW:

  • your original NSW birth certificate
  • 3 certified identification documents including a current photo ID
  • one form of ID that shows your current residential address.

For a child born in NSW:

  • the personal and contact details of the parents
  • 3 certified identification documents for each parent, including evidence of their places of birth
  • certified photo identification of each parent
  • the child's original NSW birth certificate
  • certified photo identification of the child
  • 2 other certified identification documents.

More information

  • All identification and other supporting documents must be certified by one of the following qualified witnesses:
    • Justice of the Peace
    • Notary Public
    • Legal Practitioner (holding a current practising certificate)
    • A person authorised to administer an oath under section 26 of the Oaths Act 1900.
  • Certificates are sent by the Registry of Births, Deaths & Marriages via registered post. They cannot be collected at a service centre.
  • Processing times commence when applications are received in full compliance with Registry requirements, and exclude delivery time.
  • Documents provided as proof of identity may have their authenticity verified through the online Certificate Validation Service (CertValid) and the National Document Verification Service (DVS).
