This privacy collection notice relates to Request an Internal Review of a decision about a grant application.

Service NSW is collecting your personal information to conduct an internal review of a grant application.

We may collect the following personal information:  

• First and last name 
• Postal address 
• Contact phone number 
• Email address 
• Name of the applicant 
• Your relationship to the applicant 
• Application reference number 
• Business name 
• Business address 

Service NSW uses personal information it collects about you to: 

• conduct an internal review of the administrative decision made about your application 
• contact you or an authorised third party about your dispute with the administrative decision made about your application. 

Service NSW will not disclose your personal information to anyone else unless you provide your consent, or we are required or authorized to do so by law.  

Providing us with the requested information is not required by law. However, if you choose not to provide us with the requested information, we may not be able to conduct an internal review.
