Service NSW is collecting your personal information because you have asked us to review the administrative decision made on your grant application.
Type of personal information we collect
To carry out an internal review of the administrative decision made on your application, we may need to collect the following personal information:
- First and last name
- Postal address
- Contact phone number
- Email address
- Name of the applicant
- Your relationship to the applicant
- Application reference number
- Business name
- Business address
If you are requesting this internal review on behalf of another individual, you must obtain authorisation from that person before disclosing their personal information to Service NSW. We recommend a copy of this Privacy Notice be provided to any individual that has been included in your application.
How we will use and disclose this information
Service NSW will use your personal information for the primary purpose that it was collected for, or a directly related secondary purpose. This includes to:
- verify your identity and locate your customer record
- conduct an internal review of the administrative decision made about your application
- contact you or an authorised third party about our review
- maintain and use information collected about you for internal administrative purposes, including for the purpose of our interactions with you
As part of the review, we may ask you for more evidence to demonstrate your eligibility for the grant. The privacy collection notice for the grant you applied for explains how we will handle this information.
Service NSW will not disclose your personal information to anyone else unless you provide your consent, or we are required or authorised to do so by law.
Whether you are required by law to provide us with this information
Providing us with the requested information is not required by law. However, if you choose not to provide us with the requested information, we may not be able to conduct an internal review of the decision relating to your grant application.
