Introduction
You'll need to apply for a gaming machine dealer's licence if you want to:
- manufacture and assemble gaming machines, in or on your premises
- sell and negotiate the sale of approved gaming machines (regardless of whether you manufactured or assembled them)
- service, repair and maintain approved gaming machines, in or on your premises.
You must be a corporation to apply for a gaming machine dealer's licence.
Eligibility
- The company probity forms must be completed by the principal executive officer of the corporation, or another person:
- who has knowledge of the relevant facts
- has been duly appointed by the company board to complete the forms
- whose authority has been minuted.
What you need
- the application forms:
- company probity form (part A)
- company probity form (part B)
- individual probity form.
- a recent colour passport size photo of the person completing the individual probity form
- the documents:
- Public Consultation Notice
- Premises Notice
- Newspaper Notice.
- a JP or legal practitioner to:
- witness the required affidavits
- verify identity where required.
- a photocopy of the item/s used to verify identity, such as a driver licence or a passport.
How to apply
- Email lgnsw.approvals@liquorandgaming.nsw.gov.au to request the following application forms:
- company probity form (part A)
- company probity form (part B)
- individual probity form.
- Read the 'Instructions for Completion', as set out in each form.
- Complete the 3 forms, including the checklists, and affidavits (where applicable).
- Make 2 copies of each completed form.
- Collect all the corporate and personal documents as itemised on each form.
- Download and print Public Consultation Notice – DOC.
- Complete the form, and make one copy of it.
- Gather together:
- the 3 original completed application forms
- the 2 copies of each completed application form
- all the required supporting documentation, photo/s and photocopy/s as indicated on the 3 application forms
- the original completed Public Consultation Notice.
- Lodge your application, marked 'confidential':
- in person – Market Product Approvals & Casino Operations, Liquor & Gaming NSW, Level 6, 323 Castlereagh Street, Haymarket NSW 2000, or
- by post – Market Product Approvals & Casino Operations, Liquor & Gaming NSW, GPO Box 7060, Sydney NSW 2001.
- Affix the copy of the completed Public Consultation Notice to the proposed premises immediately before or within 2 days of lodging your application.
- Download, complete and print 2 copies of Premises Notice – DOC.
- Lodge one copy with the local police and one with the local council.
- Download, complete and print 2 copies of Newspaper Notice – DOC.
- Publish the advertisement in a national newspaper and a local newspaper.
More information
- The forms must be completed in English, and any supporting documentation not in English must have a certified English translation appended.
- Ensure you complete the following, or your application may be refused by Liquor & Gaming NSW:
- affix the copy of the Public Consultation Notice to the proposed premises
- lodge a copy of the Premises Notice with the local council and local police
- publish an advertisement in a national and a local newspaper, using the Newspaper Notice form.
