Use this form to ask a question, report an issue or give feedback about a Service NSW program or service. All fields must be completed unless marked optional.

Use this form to tell Service NSW about suspected fraud involving your MyServiceNSW Account, NSW Digital ID, or any other Service NSW transaction where you believe your identity has been used fraudulently.

Important information

  • If you believe your MyServiceNSW Account has been accessed or used fraudulently, refer to How to stay cyber safe for immediate steps you can take to protect your account.
  • If you believe your driver licence details have been compromised in a data breach, refer to data breaches information for immediate steps you can take to protect your identity.
  • If you want to report suspected business grant fraud that does not involve your identity (for example, you believe a business has fraudulently applied for a disaster grant), you must use this form.
