Introduction

If you did not receive the registration certificate for your plant design or plant item, or it contains a printing error, you can apply for a replacement document from SafeWork NSW.

You can also apply if your registration certificate has been lost, damaged, destroyed or stolen.

Note: If more than one item/design registration document is required you need to pay the applicable fee for each document.

Eligibility

The registered owner of the plant item or plant design, or their authorised nominee can apply for a replacement.

What you need

  • the PDF form – 'Application to replace a plant item or design registration'
  • the registration number if known, or sufficient details to identify the registration
  • a letter of authority from the registered owner (if applicable)
  • the reason for replacement, such as:
    • how the document came to be lost, damaged, destroyed or stolen, or
    • details of the printing error
  • your payment.

How to replace

  1. Select the 'Download PDF form' button.
  2. Complete 'Application to replace a plant item or design registration – PDF'.
  3. Make your secure payment.
  4. Email or post your application, supporting documents and payment receipt number to the address on the form.

  • Credit or debit card (MasterCard, Visa, American Express, UnionPay or Diners Club)
  • Paypal.

Fees and charges

You may need to pay a merchant fee when paying by credit or debit card.
