Report erratic or unsafe driving practices

Introduction

If you’re worried about a person's medical fitness or ability to drive safely, you can share your concerns by submitting an unsafe driving report. Any information you provide in the report is confidential.

Transport for NSW has information about how health conditions can affect your licence and safe driving in NSW.

There is also a useful guide for older driver licensing.

Note: If you've witnessed dangerous driving and the driver is unknown to you, call the Police Assistance Line on  131 444.

How to report

1. Select the 'Download PDF form' button.
2. Complete the form.
3. Visit a service centre and submit your report.

You will need proof of identity to complete this process.
