Introduction

As a registered combatant, you must provide the Combat Sports Authority with updated medical documents to compete in combat sports contests in NSW. As a registered combatant, you must:

  • give the Authority a current certificate of fitness, no later than 12 months after the date of the previous certificate, and
  • give the Authority a current serological clearance, no later than 12 months after the date of the previous certificate.

Eligibility

You must be a registered combatant.

What you need

  • a MyServiceNSW Account
    • you may need details from 2 identity documents (such as an Australian driver licence, Medicare card, passport)
  • your combatant registration number
  • your MDU (Medical Document Update) number, including all digits and letters (as shown on your notice to update medical documents)
  • the PDF forms:
    • a current Certificate of Fitness completed by a medical practitioner and submitted within 28 days of the issue date on the certificate, and/or
    • a current Serological Clearance Certificate completed by a medical practitioner or a pathology service provider
      Note: if you're under 18, the date of the tests must be within the past 12 months. If you're 18 or over the date of the tests must be within the past 6 months.
  • a signed Parent/Legal Guardian Consent form for applicants under 18 years of age.

If you've been registered as a combatant before, but have changed your name, you need to inform the Authority by completing the Change of name form before completing this transaction.

How to apply

  1. Gather the required details and documents.
  2. Select the ‘Update online’ button.
  3. Log in to your MyServiceNSW Account.
  4. Enter your registration and MDU numbers, these can be found on your MDU notice.
  5. Follow the prompts to provide updated medical documents. 

Once you've submitted your application, you'll get an email confirmation.

Privacy

To update medical documents for your combatant registration with the NSW Office of Sport, you’ll need to create or log in with a MyServiceNSW Account.

By logging in you consent to sharing your name and email address in your MyServiceNSW Account with the NSW Office of Sport.

More information

  • You must take your medical record book with you to the medical practitioner when obtaining updated medical documents. The medical practitioner is required to complete the approved Certificate of Fitness and/or the Serological Clearance Certificate forms as well as the Certificate of Fitness and Serological Clearance Certificate history pages contained within your medical record book.
  • You must take your medical record book to all contests you compete in and show it to the attending medical practitioner.
  • If you need to update your details, go to Change name or contact details on a combatant registration.
  • To check your combatant registration number and details, visit Verify.licence.
