Introduction
As a registered combatant you can apply online to replace your medical record book if yours has been damaged, lost, stolen, not received or is full.
What you need
- a MyServiceNSW Account
- you may need details from 2 identity documents (such as an Australian driver licence, Medicare card, passport)
- your combatant registration number
- a completed Statutory Declaration
Note: the completed statutory declaration must be signed by a Justice of the Peace, Notary Public or Legal Practitioner. It is not required to be provided for replacement of a full book. - your payment.
If you've been registered as a combatant before, but have changed your name, you need to inform the Authority by completing the Change of name form before completing this transaction.
How to replace
- Gather the required details and documents.
- Select the ‘Replace online’ button.
- Log in to your MyServiceNSW Account.
- Enter your registration number.
- Follow the prompts to apply for a replacement medical record book.
Once you've submitted your application, you’ll get an email confirmation and payment receipt (if applicable).
What happens next
Replacement medical record books will be sent to your recorded postal address within 10 business days of your application.
If you are competing as a combatant in a combat sports contest within the next 3 weeks, contact the Combat Sports Authority of NSW to discuss alternative arrangements by:
- calling 13 13 02
- emailing combatsports@sport.nsw.gov.au.
Privacy
To apply for a replacement medical book with the Combat Sports Authority of NSW (NSW Office of Sport), you’ll need to create or log in with a MyServiceNSW Account.
