Service NSW in partnership with Create NSW is collecting your personal information to assess your eligibility as a provider for the Creative Kids program. This includes information about your business, the nominated contact person for your business and employees or volunteers who work for you. There is a separate privacy collection notice for parent applicants.
Why are we collecting your personal information
Service NSW is collecting your personal information for the purposes of processing and determining eligibility as a provider for the Creative Kids scheme. This may include disclosing the contents of your application, and any other additional information you provide, to Create NSW and other third parties outlined in this notice for further assessment and review.
What personal information will we collect
Service NSW will only collect such information that is reasonably necessary to verify the identity of providers and deliver and evaluate the Creative Kids scheme. To verify the identity and evaluate providers, the following personal information may be collected, including name, date of birth, proof of identity documents, contact details such as address, email and phone number, qualifications, Working with Children Check information. Business information such as ABN, ACN, insurance and bank account details are also collected.
How will we use and disclose your information
Service NSW will use your personal information for the primary reason that it was collected or a directly related secondary purpose. This will include contacting you about your application as a provider for the Creative Kids program.
Service NSW uses your personal information to conduct auditing, investigation and compliance activities to assess whether the eligibility criteria and Terms and Conditions have been and are being met. We may also investigate any allegations of fraud or failure to comply with Terms and Conditions and to assess the financial integrity of the scheme. This may involve sharing information with Create NSW, or other public or private entities, for example, NSW Police, the Australian Tax Office, or their delegated agent, for this purpose.
Service NSW also maintain and use information about you for our internal administrative purposes, including for the purpose of our interactions with you.
We may also disclose your personal information to third parties in connection with the administration and management of the Creative Kids program. This includes to:
- Create NSW for the purpose of administering the program
- Australian Securities and Investment Commission and the NSW Incorporated Associations Register (managed by the NSW Office of Fair Trading), for the purposes of verifying identity documents and details about providers and their office bearers
- The Document Verification Service to verify any proof of identity documents
- Working With Children Check numbers with the Office of the Children's Guardian
- The ‘Business Finder’ page of the Service NSW website which will publish business details and contact information for providers
- A provider's nominated contact person will be able to access provider details (including names and contact details of other office bearers, delegates and key contacts) via the provider's MyServiceNSW Business Profile account.
Create NSW may share de-identified information for research and statistical purposes. This may include sharing certain information to evaluation consultants for the purposes of program evaluation.
Are you required by law to provide us with this information
Providing us with the requested information is not required by law. However, if you choose not to provide us with the requested information, we may not be able to process your application as a provider.
