Why we are collecting your personal information

Service NSW is collecting your personal and health information to determine your eligibility for the Pre-IVF Fertility Testing Rebate.

Types of personal and health information we collect

In order to determine your eligibility for the rebate, Service NSW may collect a range of personal and health information about you, including:

  • your name, signature, and contact information
  • your date of birth
  • your identity document details 
  • additional evidence and documentation as required such as to provide evidence of a name change
  • banking details
  • the name and signature of the other patient (partner or gamete donor), if applicable
  • your specialist’s name and signature
  • information given in the rebate form about relevant pre-IVF testing and any receipts for relevant patient related to the rebate claim (we do not request test results as part of the application). 

How we collect your personal and health information

We collect personal and health information directly from you wherever possible. Information may be collected indirectly where an application is made by the other patient (partner or gamete donor) on your behalf. 

Where a rebate application is submitted by you as the applicant for tests completed by the other patient  you must obtain the consent of the other patient to submit a claim using their receipts and personal information. 

Service NSW will only collect personal information that is reasonably necessary to process your application for the rebate or for related purposes.  

How we will use and disclose this information

Service NSW uses personal information it collects about you to determine your eligibility for the Pre-IVF Fertility Testing Rebate. Service NSW may also use your personal and health information for the following purposes:

  • to process a payment to you if your application is successful
  • verify your identity documents with the Document Verification Service
  • to investigate or manage a complaint or incident
  • carry out auditing and compliance activities to confirm that the relevant eligibility criteria have been met or to detect and investigate fraud
  • assist you with any enquiries or complaints you may have in relation to your application
  • evaluate and report on the program using de-identified or aggregated data (information that does not identify you)
  • to contact you for feedback on your experience relating to the rebate.

Service NSW maintains and uses information collected about you for internal administrative purposes, including for the purpose of our interactions with you.

Service NSW will not disclose your personal or health information to anyone else unless you provide your consent or we are required or authorised to do so by law.

Whether you are required by law to provide us with this information

Providing us with the requested information is not required by law. However, if you choose not to provide us with the requested information, we may not be able to determine your eligibility for the rebate.
