Introduction

If you're the birth mother of a child born in NSW, and you want to list your same sex spouse or de facto's details on your child's birth certificate, you can lodge an application at a service centre, or post the required documentation to the Registry.

Eligibility

  • the birth must be registered in NSW
  • the mother must have been married to, or in a de facto relationship with the birth mother at the time of the fertilisation procedure, and have consented to the procedure.

What you need

  • the personal and contact details of the birth mother
  • the personal and contact details of the other mother
  • the child's personal details
  • 3 certified forms of identification for each mother, as specified on the form
  • original copies of birth certificates or extracts previously issued by the Registry
  • marriage details (if applicable)
  • your payment
  • the PDF form – 'Adding a parent's details to a birth registration'.

How to apply

  1. Check the eligibility requirements.

  3. Gather all the required documentation.
  4. Visit a service centre and lodge the information in person, or mail the application to:

  • If a father is recorded on your child’s birth certificate, you'll need a court order to add a mother’s details.
  • Make sure you include the original copies of birth certificates or extracts previously issued by the Registry, when you lodge your application.
  • Certificates are sent by the Registry of Births, Deaths & Marriages via registered post. They can't be collected at a service centre.
  • Turnaround times commence when applications are received in full compliance with Registry requirements, and exclude delivery time.
  • If you change your mind, your application fee will not be refunded.
  • Documents provided as proof of identity may have their authenticity verified through the online Certificate Validation Service (CertValid) and the National Document Verification Service (DVS).
