Introduction
As the holder of a PWC licence you'll need to notify Transport for NSW of any changes to medical/physical conditions, including eyesight, that might cast doubt on your fitness to operate a PWC.
To change the conditions on your PWC licence you can visit a service centre, or submit your documentation by mail.
Note: Changes to eyesight conditions will require a replacement licence.
What you need
- your proof of identity
- Adjustment of Records – Maritime Customers Only – PDF
- your PWC licence.
In addition, if adding or removing an eyesight condition, you may need:
- a copy of your current Australian driver licence indicating whether or not you have an eyesight condition:
- if removing a condition, a medical report/certificate or other written advice from a doctor, specifically stating that the condition is no longer required, and showing your eyesight standard
- to undergo an eyesight test at the service centre
- Application for a Personal Watercraft Driving Licence – PDF
- Application for a Replacement Product – PDF
- two colour, passport-size photos (if applicable).

- Make sure you have all the required documentation, and:
- visit a service centre and provide the information in person, or
- mail the documentation to:
