Change the details on a motor vehicle tradesperson certificate

Introduction

If you hold a motor vehicle tradesperson certificate and your name or contact information has changed, you need to notify NSW Fair Trading.

To look up your certificate number and details, visit Verify NSW.

What you need

• your certificate number
• evidence of your name change, such as a change of name or marriage certificate (if applicable)
• the PDF form – 'Change of name or contact details'.

How to change your details

1. Select the 'Download PDF form' button.
2. Complete the form, including the declaration.
3. Attach your supporting document for change of name (if applicable).
4. Submit the form and attachment (if applicable):
   • at a Service NSW Centre, or
   • by email using the address on the form.
