Replace a motor vehicle repairer licence

Introduction

If your motor vehicle repairer licence has been lost, damaged, or destroyed, you can apply for a replacement via email, or in person at a Service NSW Centre.

What you need

• the PDF form – 'Application for Replacement Licence or Certificate'
• details of the licence (including licence number)
• contact details of the licence holder
• your fee payment receipt number.

How to replace

1. Select the 'Download PDF form' button.
2. Complete the form and submit it:
   • at a service centre, or
   • by email using the details on the form.
