Replace a commercial mooring licence

Introduction

If your commercial mooring licence certificate is lost, damaged or destroyed, you can order a replacement at a Service NSW Centre, or by post.

What you need

• the PDF form – Application for Maritime Replacement Products
• proof of identity
• your payment.

How to replace

1. Select the 'Download PDF form' button.
2. Make sure that you have all the required documents.
3. Visit a service centre and submit your application in person, or post it to the details on the form.
