Introduction
As a registered industry participant and promoter, you can apply online to replace your identity card if yours has been damaged, lost, stolen or not received.
What you need
- a MyServiceNSW Account
- you may need details from 2 identity documents (such as an Australian driver licence, Medicare card, passport)
- your registration number.
If you've been registered as an industry participant and promoter before, but have changed your name, you need to inform the Authority by completing the Change of name form before completing this transaction.
How to replace
- Select the ‘Replace online’ button.
- Log in to your MyServiceNSW Account.
- Enter your registration number.
- Follow the prompts to apply for a replacement identity card.
What happens next
- You'll get a replacement industry participant identity card mailed to your recorded mailing address within 10 business days of submitting your application.
- Please ensure your recorded mailing address is current.
Privacy
To apply for a replacement identity card with the Combat Sports Authority of NSW (NSW Office of Sport), you’ll need to log in to your MyServiceNSW Account.
