Transport for NSW customer number
To complete and submit your Medical Certificate using the online portal your doctor will need either:
- your NSW Driver Licence, or
- your Transport for NSW customer number.
You will have a Transport for NSW customer number if you have any of the following:
- an MPS permit previously issued
- NSW driver or rider licence
- a Transport for NSW issued photo card (for example, NSW Photo Card)
- vehicle registered in NSW.
If you need to find your customer number, you can call us on 13 77 88, or visit a service centre with one of your Transport for NSW documents.
If you do not have a customer number you can Apply for a customer number with Transport for NSW.
Link your MyServiceNSW Account to Transport for NSW
To make your mobility parking permit request online your MyServiceNSW Account must be linked to Transport for NSW.
To link your account you will need a:
- NSW Driver Licence, or
- Photo Card.
- Log in to your MyServiceNSW Account.
- Go to MyServiceNSW Account services.
- If not already added, add the 'NSW driver licence and vehicle registrations' service.
Update details with Transport for NSW
Once you've linked your MyServiceNSW Account to Transport for NSW, check your details are up to date by going to the Personal details section of your MyServiceNSW Account:
- Check that your given name and family name match your Transport for NSW documents (for example, driver licence, Photo Card, vehicle registrations, current mobility parking permit). If required, update your details.
- Link your email to Transport for NSW by entering in the email you would like to use across both Service NSW and Transport for NSW. Click the checkbox to link with NSW Driver Licence and vehicle registrations.
- Check that both your residential and mailing address are correct. If not, update in the Address section, select Continue and on the next page click the checkbox to link with NSW Driver Licence and vehicle registrations.
Representatives – applying for someone else
If someone you know is representing you at a service centre they should:
- bring their own proof of identity documents as well as yours, and
- fill in the representative declaration in the application form, or
- bring an original or certified copy of a Power of Attorney document.
Note: the powers described in the Power of Attorney document will need to cover transactions of this kind.
Photos – how they work
Your permit includes a photo to stop misuse.
The photo will be kept on file by Transport for NSW. If you already have a Transport for NSW profile (for example if you have a NSW Driver Licence or Photo Card) you will generally already have a photo on file.
The photo on file
- can be reused if it is still valid
- may need to be re-taken at a service centre if it is too old.
If you have a photo exemption, different steps may apply.
Photo exemptions
You're automatically exempt if you're under 16. If you're 16 or over photo exemptions must be recorded before you begin online mobility parking permit requests.
You can request a photo exemption if any of the following apply to you:
facial disfigurement
inadequate muscular control to meet photo requirements
mostly bedridden due to advanced age or terminal illness
severe disability that stops you from attending a service centre
mobility restricted by medical equipment.
To apply for a photo exemption, send a letter of request along with a completed Mobility Parking Scheme Application – PDF form and a supporting letter from your doctor, to:

If approved, you'll be sent a letter of exemption.
For online applications your doctor will also need to submit the online form after you've got your letter of exemption.
For in person applications at a service centre bring your exemption letter and completed mobility parking form.
Using your permit
You must display both your MPS permit and Australian Disability Parking permit (large purple card) to get parking concessions. Refer to How to display your mobility parking permits.

Permit holder
If your permit holder is lost or damaged you can get a replacement for free at a service centre.
