Change the medical conditions on a licence

Introduction

If you need to change, add or remove the medical conditions on your driver licence (for example Transport for NSW may have requested that you have a medical review as part of a licence application), you'll need to visit a Service NSW Centre.

What you need

• your proof of identity
• your current NSW Driver Licence
• your medical report
• the relevant PDF form:
  • 'Licence Application'
  • 'Licence Renewal Application'
  • 'Mobility Parking Scheme Application'.

How to change

1. Download and complete the relevant form:
   • Licence Application – PDF 
   • Licence Renewal Application – PDF
   • Mobility Parking Scheme Application - PDF (for individuals)
2. Attach your supporting documentation.
3. Visit a service centre and submit the information.
