Change name or contact details on a combatant registration

Introduction

You can change the contact details on your combatant registration (address, email, mobile or telephone number) online. We’ll use these details to contact you about your registration.

To change your name, complete the Combat Sports Authority Change of name form.

If you're also registered as an industry participant or promoter, you'll also need to update your contact details on your industry participant and promoter registration, go to Change name or contact details on an industry participant and promoter registration. 

Changes to your MyServiceNSW Account.

Any changes you make to the contact details on your combatant registration will not automatically update on your MyServiceNSW Account.

You should continue using your MyServiceNSW Account email address to access the Combat Sports Registration System. If you need to update your MyServiceNSW Account email address, go to Manage your details with your MyServiceNSW Account. 

What you need

To change your contact details:

• a MyServiceNSW Account
  • you may need details from 2 identity documents (such as an Australian driver licence, Medicare card, passport)
• your registered contact details
• your new contact details.

To change your name:

• the Combat Sports Authority Change of name form
• your contact details
• your registration number
• one form of identity document that contains a photo and has been issued in your new name
• a copy of one of the following change of name documents:
  • marriage certificate(s) issued by the NSW Registry of Births, Deaths & Marriages or, if married elsewhere, the marriage certificate issued by the celebrant or church
  • change of name certificate issued by the NSW Registry of Births, Deaths & Marriages
  • full birth certificate showing your name at birth and your new name (extracts and Commemorative certificates are not acceptable)
  • divorce order
  • deed poll registered with the relevant authority
  • proof that your change of name has been registered with NSW Land Registry Services

How to change

To change your contact details if you chose document verification to verify your identity:

1. Select the 'Change online' button.
2. Log in to your MyServiceNSW Account.
3. Follow the prompts to change your contact details.

To change your contact details if you chose manual verification to verify your identity, contact the Combat Sports Authority of NSW on 13 13 02.

To change your name:

1. Gather the required information and documents.
2. Complete the Combat Sports Authority Change of name form.
