Introduction
If you’re temporarily interstate or overseas and your NSW driver licence card has been lost, stolen or destroyed, you can order a replacement licence online.
If you're unable to replace online or need the licence to be sent outside NSW, you'll need to complete a photo-kit form and send it by post. It may take 6 to 8 weeks for the form to be processed and the replacement licence sent.
Eligibility
You can use a photo-kit form only if your licence:
- is not a temporary paper licence
- is not suspended or cancelled.

- Photo-kit for a NSW Licensee Temporarily Interstate, or
- Photo-kit for a NSW Licensee Temporarily Overseas

To complete the form you will need to provide:

Change of name
- You must also provide a change-of-name document that has been witnessed and certified if your proof of identity documents are in an old name because of a name change.

- You cannot ask for a name change using a photo-kit form.
