Register a change of name

The NSW Registry of Births, Deaths & Marriages is responsible official changes of name. There is a fee for this service.

Before you apply, you may want to read Registering a change of name policy. The policy explains the restrictions around registering a name in NSW, and why some names may not be accepted by the Registry.

You can apply to register a change of name online. The online form can be used by adults for themselves, or by parent(s), guardian(s) or caseworkers on behalf of a child.
