Replace a motor vehicle repairer licence

Introduction

If your motor vehicle repairer licence has been lost, damaged, or destroyed, you can apply for a replacement via email, or in person at a Service NSW Centre.

What you need

• the PDF form – 'Application for Replacement Licence or Certificate'
• details of the licence (including licence number)
• contact details of the licence holder
• your payment.

How to replace

1. Select the 'Download PDF form' button.
2. Complete the form.
3. Make your payment and enter the receipt number on the form.
4. Complete the declaration.
5. Submit your completed form at a service centre, or email it to the address specified on the form.
