Request access to personal records held by Transport for NSW

Introduction

To request information about your NSW Driver Licence, motor vehicle or trailer complete the PDF form. 

Only the licence holder can be given licence details.

Restrictions

Only registration information relating to a known registered operator can be provided.

Personal information about a current or previous operator will not be provided as part of a vehicle information enquiry.

What you can ask for

You can ask for:

• confirmation of licence details
• confirmation of registration details
• vehicle listings
• name or address history.

Driving record

If you want a copy of your driving record – either uncertified or certified – you can make a  request online.

Notice of disposal

To ask for details of a notice of disposal for a vehicle you sold contact us.

We can only provide this information to you if you lodged the form when you sold the vehicle.

What you need

To make a request you will need:

• your proof of identity
• your payment
• the PDF form – 'Access to Own Personal Records Application'.

How to request

1. Download the PDF form – Access to Own Personal Records Application.
2. Complete the form.
3. Attach copies of your proof of identity documents to the form.
4. Submit your request:
   • in person at a Service NSW Centre, or
   • by email or mail using the details on the form.
