Request access to personal records held by Transport for NSW

Introduction

If you require written information about the status of your NSW Driver Licence, or details on your motor vehicle(s), you can submit a request to Transport for NSW.

Only registration information relating to a known registered operator can be provided.

Note: Your driving record, either uncertified or certified can be requested online.

What you need

• your proof of identity
• your payment
• the PDF form – 'Access to Own Personal Records Application'.

How to request

1. Select the 'Download PDF form' button.
2. Complete the form.
3. Submit your request:
   • in person at a Service NSW Centre, or 
   • by email or mail using the details on the form.

More information

• Only the licence holder can be given licence details.
• Under no circumstances can personal information about a current or previous operator be provided as part of a vehicle information enquiry.
