Change address and contact details on a qualified supervisor certificate

Introduction

If you hold a qualified supervisor certificate and NSW Fair Trading has your email address or mobile phone number on record you can update your address or contact details online.

Only your address and contact details can be changed online.

Change of name

If you want to change your name on your certificate you need to visit a service centre with a copy of the name change document such as a marriage certificate. Find out more about change of name.

What you need

To update your details you need:

• your name and licence number
• your new contact details.

How to change your details

1. Select the 'Change online' button.
2. Follow the prompts to update your details.

If you cannot change online:

1. Download and complete the Change of address/contact details – PDF.
2. To submit the form send it to the email address on the form or go to a service centre.
