Introduction

If your qualified supervisor certificate has been damaged, lost or stolen, you can apply for a replacement at a Service NSW Centre.

Note: A replacement can only be issued for a current certificate.

How to replace your certificate

  1. Check your qualified supervisor certificate is current at Verify NSW.
  2. Download and complete the 'Application form – Replacement licence, certificate or permit – PDF'.
  3. Take your completed form and payment to a service centre.

Note: Your application may take longer to assess if there is missing or incomplete information.

Payment and fees

When you apply you need to pay a fee.

To pay at a service centre, check the payment options available.

Some service centres do not accept cash payments.
