Introduction
If your qualified supervisor certificate has been damaged, lost or stolen, you can apply for a replacement at a Service NSW Centre.
Note: A duplicate can only be issued for a current certificate.
How to replace your certificate
- Check your qualified supervisor certificate is current at Verify NSW.
- Download and complete the PDF form 'Application for duplicate licence, certificate or permit'.
- Take your completed form to a service centre.
Payment and fees
You will need to pay a fee for this service. For current fees visit NSW Fair Trading Fees for contractor licences, supervision and tradesperson certificates.
You cannot pay with cash at some Service NSW Centres. Check our payment methods page for the list of locations where cash is not accepted.
