Replace an individual contractor licence

Introduction

If you hold an individual contractor licence and it's been damaged, lost or stolen, you can apply for a replacement licence at a Service NSW Centre.

How to replace your licence

1. Check your licence number and details at Verify NSW.
2. Select the 'Download PDF form' button.
3. Complete the Application form – Replacement licence, certificate or permit – PDF.
4. Take your completed application form and payment to a service centre.

Note: Your application may take longer to assess if there is missing or incomplete information.

Payment and fees

When you apply you need to pay a fee.

To pay at a service centre, check the payment options available.

Some service centres do not accept cash payments.
